If you enroll in the Core or Core Plus medical plan option, you can earn contributions from UBS to your Health Savings Account (or Health Reimbursement Account, if applicable) by completing a variety of wellness activities.
- If you enroll yourself only: Up to a maximum of $4001, 2
- If you cover any family member: Up to a maximum $8001, 2

Find everything you need to stay on top of your health and track your wellness activities to earn UBS contributions to your HSA (or HRA) on the MyActiveHealth3 website: http://myactivehealth.com/ubs.
The site has:
- Wellness incentives offered by UBS
- A place where you can keep track of what you’ve earned
- News and information about many health and wellness-related topics
You and your spouse/partner and any children over age 18 can register and log on to the site.2
The ActiveHealth site is confidential and private. Your personal health information is not shared with UBS.
You can complete a variety of different healthy actions – from health assessments and biometric screening to registering for our Telemedicine offering to participating in run or walk.
Once you've completed the activity and tracked it in MyActiveHealth, ActiveHealth will notify UBS of the completion (no other details4). If the activity requires notification from a third party (i.e. preventive exam, attend a webinar, etc.), ActiveHealth will mark the activity as “complete” and notify UBS once they’ve received notification from the third party.
UBS's contribution to your Health Savings Account or (Health Reimbursement Account, if applicable) should typically occur within two months, from that notification.
We want to support you in making informed choices with respect to healthcare.
We also want you to understand that your privacy in matters of health and healthcare is paramount to us and is protected by federal law.
- In any health or biometric assessment and any other similar assessments as part of this, UBS is not requesting or receiving any genetic information as part of the wellness incentive. Your personal information and results are yours and yours alone, although you may wish to share them with your doctor.
- UBS will not see your or your family’s individual information, health status or the details of your participation any Wellness programs.
- Our plans will report overall claims information and wellness participation results to UBS (no individual data) so we can track the health risks that challenge our population as a whole, as well as the results of our wellness programs. This gives us an opportunity to develop and modify our programs and resources so they are most beneficial to our employees.
- With respect to Wellness Programs, UBS will only be notified by ActiveHealth of completion of a given activity, not of any results.
Important notes
1. You are only eligible to earn up to the maximum wellness contribution (based on your coverage level), no matter how many wellness activities you participate in. $400 for employee only coverage. $800 if you cover any family member.
2. Employees and their covered spouses/domestic partners may earn UBS contributions to the employee’s HSA by completing wellness activities via the MyActiveHealth portal; dependent children cannot earn incentives. If you cover yourself and a child or children, but not a spouse/domestic partner, you will have access to complete more wellness activities via MyActiveHealth in order to earn the maximum $800 incentive. Dependent children over the age of 18 can, however, register and use MyActiveHealth. They just don't receive the incentive.

Having a Health Savings Account (HSA) through UBS provides you an opportunity to earn or receive contributions from the firm. But if you don't enroll in UBS's Core or Core Plus plan, you cannot open an HSA through UBS. Under IRS rules, you cannot contribute to a HSA unless you are covered under a high deductible health plan or consumer directed health plan (CDHP).
UBS contributions to your Health Savings Account are contingent upon your being employed by UBS on the date the contributions are made to the account. UBS reserves the right to change or terminate its benefit plans at any time, including, without limitation, changing the amount of any UBS contributions to HSAs.
